I would like to establish a form where input data is restricted to select field/cells.

I am interested in generating a PDF document where the user enters information only in select filed/cells.  The remainder of the document remains locked.

Example:

Name

Address

City   State   Zip Code

I would like the user to hit the tab button and move thru these field and input their information.   Any assistance would be appreciated.
